A coroner’s inquest in Singapore has brought to light the tragic circumstances surrounding the death of Hla Thein Aung, a 37-year-old construction worker from Myanmar. Hla Thein Aung fell four storeys while painting a condominium at Waterfront Isle along Bedok Reservoir Road. This incident, which took place on January 12, 2023, marked the first workplace fatality of the year.

Critical Safety Lapses

The investigation revealed that Hla Thein Aung had removed crucial safety equipment, including his helmet and boots, shortly before his fatal fall. He was found not wearing a helmet and not connected to any safety line, contrary to standard safety protocols. His shoes and safety line were later discovered on the first-floor staircase landing, while his gloves and helmet were found on the fourth-floor parapet. Although he was wearing a safety harness, it was loosely fitted.

Probable Influence of Alcohol or Drugs

The inquiry also uncovered that Hla Thein Aung might have been under the influence of alcohol or drugs. A toxicology report indicated the presence of substances, including nitrazepam, in his system, which could have impaired his motor skills and balance. His supervisor noted that Hla Thein Aung frequently took medical leave, especially on Mondays and Tuesdays, and occasionally consumed alcohol, though not during working hours.

Company’s Response and Regulatory Action

Following the incident, the Ministry of Manpower (MOM) issued a stop-work order to ISOTeam C&P, Hla Thein Aung’s employer. The company was barred from employing new foreign workers for three months due to poor risk controls. In response, ISOTeam C&P revised its safety system, ensuring a “100 per cent tie-off” policy where workers must always be secured to a fall protection system while working at elevated heights.

The death of Hla Thein Aung was the first of 14 workplace fatalities reported in the first half of 2023. This figure represents a decrease from the previous year, yet highlights ongoing challenges in workplace safety, particularly in high-risk sectors like manufacturing and construction.
